Please complete and sign this form before your first appointment at Hanna Madison Salon, 6835 Midland Commons Blvd, Building 1, Suite C, Columbus, GA 31909. It takes about two minutes and helps us plan a safe, beautiful service for you.

Client Service Release

By signing below, I agree to the following:

Voluntary services. I am requesting hair and beauty services from Hanna Madison Salon and its stylists of my own free will.
Known risks. I understand that chemical and heat services, including hair color, lightening, highlights, keratin and smoothing treatments, extensions, waxing, and makeup, carry some risk. This may include allergic reaction, skin or scalp irritation, hair breakage or dryness, and color results that differ from what I expected, especially on previously colored, damaged, or chemically treated hair.
Full disclosure. I have told the salon about any allergies, sensitivities, medical conditions, medications, pregnancy, and past chemical services that could affect my results. I understand the salon relies on this information, and I will update my stylist if anything changes.
Patch and strand tests. I understand a patch or strand test may be recommended before chemical services. If I choose not to have one, I accept responsibility for any reaction or result that a test may have revealed.
Aftercare. I agree to follow the aftercare instructions my stylist gives me. I understand results depend in part on at home care and the products I use.
Release. To the fullest extent allowed by Georgia law, I release Hanna Madison Salon, its owners, stylists, and staff from claims arising from services performed with reasonable care and in line with the information I provided. This release does not cover gross negligence or willful misconduct.
Concerns. If I am unhappy with a service or notice any reaction, I will contact the salon within 7 days so the team can review it with me.
Minors. If the client is under 18, the parent or guardian signing below accepts these terms on the client's behalf.
